FAQ's
Q. Must I have my child's immunization records to register for Hamlin Park Day Camp?
A. We can't accept any registrations without immunization records or a written and signed statement from the parent or guardian stating the reason (due to their religious beliefs, immunosuppression, serologic immunity, medical, etc.) the child is exempt.

A. You will need to complete the Health questions for both camps but you only need to upload the immunization records once. The immunization records will be linked to your child's account.

A. No. We cannot transfer registrations to friends.
Complete the Online Health Form Registration for 2024!
- You must complete your child's health and immunization information for every session registered, you only need to do it once per child. If you are also registering for JP Nicely Day Camp you will need to complete the health and immunization information again for that program. You will be required to upload a copy of your child's immunization record to complete the health form.
- Simply complete the "Online Health/Immunization Form" for each child you are registering, before you register them for Day Camp.
- It is highly recommended that you complete the health and immunization form before registration opens. Note: Day Camp registration begins April 9th for Residents, April 22nd for Non-residents. You can register your health information at any time.
- There is no charge associated with this process, complete as you would any other program, you will be "charged" $0.00. Registrations will not be accepted without this information.
